Tensions

Description

Returning older adults at home safely vs. freeing up hospital beds

Case managers strive to ensure the safety of older adults in their homes. However, they perceive pressure from the hospital staff to swiftly arrange home care services for a prompt discharge of older adults.

Delegating care to “partners” vs. managing the quality of services provided by partners

Case managers should only be minimally involved when care is provided by a partner. However, in practice, they end up overseeing the quality of the services as well as dealing with scheduling issues.

Meeting organisational requirements vs. spending more time providing care for older adults

Case managers must meet organisational requirements (e.g., up-to-date older adults’ file, up-to-date reporting of the clinical work they do through statistics, up-to-date mandatory assessment). However, in meeting these organisational requirements, they are reducing the time they are directly involved with older adults, thus the quality of care they can provide.
