Diseases were restricted by our eligibility criteria to four major groups of NCDs: cardiovascular diseases, cancers, respiratory diseases and diabetes. Cancers were further restricted to the top five cancers with the greatest disease burden for each sex. The category other in panel B consists of combinations of public and private provider types not otherwise listed
