Described connecting children with neurodisability and their families to supports and services in their community

Described a broad concept that was not specifically related to connecting children and families to services (e.g., family-centred practice)

Context

Described navigation and navigation-type resources, supports and activities relating to connecting children with neurodisability and their families to community-based pediatric services, including hospital-to-home/community-based services and across community-based agencies

Described navigation and navigation-type resources, supports and activities relating to connecting children with neurodisability and their families to only within-hospital pediatric services or to adult services (e.g., housing, employment)
