Enrol community members into the NCD portal

Adults with blood pressure  ≥ 140/90 or random blood glucose ≥140 mg/dl referred to medical officer at nearest facility (level 2) for confirmation and initiation of treatment

Level 2: Primary Health Centre

Confirmatory diagnosis of hypertension and diabetes of suspected cases referred from sub-centre

Routine management of adults with stable hypertension and diabetes

Generate and update eCRF

Use CDSS to develop treatment plan and determine need for up-referral

Level 3: Community Health Centre

Run dedicated NCD clinics

Routine management of adults with stable hypertension and diabetes
