Physician or practice member identifies and refers at risk patient, usually during visit

Physician identifies and refers at risk patient to the care manage to call the patient back later; or patient is called by the care manager based on risk adjusted list

Communication

Many types of communication: electronic medical record, ad hoc, huddles, regular meetings

Fewer types of communication: electronic medical record, monthly meetings, none at all

Team-ness

Extension of physician practice; care manager does what is needed

Separate resource; care manager is an agent of the PO
