Pathways from patient stories. Each of the three pathway steps has a color for orientation. When relevant, the factor within this pathway is chosen by filling it in. This process could allow for gradation in the strength of the factor as factors of critical importance could be highlighted darkly while factors present, but of lesser importance in explaining the potentially preventable hospitalization, could be highlighted at a lower color value.
