  2. Note: MPR of Amlodipine was 0.42 for LH; MPR of Erythromycin tab was 0.61 for CPA, 0.96 for MCD, and 1.21 for LH; MPR of Fluoxetine was 0.50 for CPA and was not procured by MCD, MSO, RML/SH.
