Responses are reclassified into categories and exclude patients with pending appointments, who were waiting for the referral, needed to reschedule their appointment, or were referred at the time of the survey. Those uninsured for any period in the last 12 months include respondents who reported being uninsured at the time of the survey and currently insured respondents who reported being uninsured at any point in the last 12 months. Those with Medicaid include respondents who reported having Medicaid coverage in each of the last 12 months
