The sampling process of Survey Participants. We first extracted 1769 township inpatient medical records with a diagnosis of respiratory diseases during 2012 and 2014 in Qianjiang district. Physicians in township and county hospitals evaluated those cases and found 1242 cases that could be treated appropriately by township hospitals. We then conducted telephone surveys and household surveys among the 1242 cases to get patients’ individual, social and disease information which cannot be found in medical records. Finally, 832 cases were participated in these surveys, the response rate was 66.99%
