  2. Comorbidity: Elixhauser comorbidity were generated for the 12 months before admission using inpatient and physician claims from MEDPAR, Outpatient Statistical Analysis Files, and Carrier files.
  3. Included only patients not enrolled in HMO and with both Medicare Parts A and B for the entire 12 months before admission. Excluded discharged to other acute care hospitals.
