Note: Four of the PCIs did not mention the action taken by the pharmacist to resolve the issues and hence the total PCIs in Table 4 is only 702*. However, the total number of PCIs which involved the GPs and the patients was only 700 as two of the PCIs which required action by the clinic staff or the caregiver are not shown in Table 4.
