  2. Congestive heart failure, diabetes mellitus, chronic obstructive pulmonary disease, rheumatoid arthritis, or cancer.
  3. Peripheral vascular disease, cerebrovascular disease, dementia, peptic ulcer disease, hemiplegia, renal disease, or liver disease.
  4. §Two or three-vessel disease with stenosis of the proximal left anterior descending (PLAD) artery.
  5. Two-vessel disease with no stenosis of the PLAD artery or one-vessel disease with stenosis of the PLAD artery.
