Patient Safety Committee

· Review all incidents reported over prior 24 hours.

Everyday

Ward nurse managers (4), safety manager (1), attending physicians (3.ER, surgery, and internal medicine), pharmacist (2).

· Provide immediate feedback to appropriate staff.
