  1. Oncologists included medical and radiation oncologists. Declined participation includes active declines, and no responses. Two developers, who were not on our recruitment lists, volunteered to participate in the study. One developer, who also held cardiology credentials was grouped into cardiologists. One oncologist, who was also a policy-maker, was grouped into policy/decision-makers. Participation rate = Agreed to participate/Total approached; Recruitment efficiency = Data received/Total approached.
