From: Determining level of care appropriateness in the patient journey from acute care to rehabilitation
Outcome of the 123 patient episodes referred for rehabilitation assessment. 180 patients were seen only once by the rehabilitation team, 35 had only one review, seven had 2 reviews and one patient required 3 reviews. Of patients requiring one or more further reviews, the mean time between initial consultation and the last review was 6.3 days (median of 5 days). 2Outcome of the re-review was that of the last re-review.
